ABSTRACT

OBJECTIVE@#To compare the assessment results of the Children Neuropsychological and Behavioral Scale-Revision 2016 (CNBS-R2016) between young children with autism spectrum disorder (ASD) and global developmental delay (GDD, without ASD) and to explore whether CNBS-R2016 could be helpful to early identification of ASD.@*METHODS@#A total of 260 ASD and 371 GDD children aged 18-30 months were enrolled to finish the assessment of CNBS-R2016. The development quotients (DQs) of the five domains of CNBS-R2016 including gross motor, fine motor, adaptability, language and social behavior were compared between the two groups. The receiver operating characteristic (ROC) curve was used to evaluate the value of the autism-predicted domain in identifying ASD and GDD.@*RESULTS@#The DQs of all the five domains in the ASD group were lower than those in the GDD group (P<0.05). The language DQ and total DQ of the ASD group had a negative correlation with the score of the autism-predicted domain (r=-0.566, -0.552 respectively, P<0.01). When the cut-off value of the autism-predicted domain was 10.5, the largest area under the ROC curve was 0.835, and the sensitivity and specificity for the diagnosis of ASD were 0.750 and 0.798 respectively.@*CONCLUSIONS@#The development of ASD children aged 18-30 months is worse than that of GDD children. CNBS-R2016 may be helpful to distinguish ASD from children with developmental delay.

ABSTRACT

BACKGROUND:The traditional two-dimensional culture system has been widely used in the in vitro culture of human tissue stem cells,but it cannot really simulate the three-dimensional physiological microenvironment in the body, which is not conducive to the study of the biological behavior of human stem cells. OBJECTIVE: To detect the effect of the bioactivity of Col-Tgel in human hematopoietic stem cells (HSCs) and mesenchymal stem cells (MSCs)in vitro and in vivo,by constructing a three-dimensional culture system stimulating the physiological microenvironment of the body. METHODS:(1)In vitro co-culture:Green fluorescent protein labeled MSCs(MSCs-GFP)and human umbilical cord blood CD34+cells were co-cultured in Col-Tgel for 3 days (three-dimensional culture group). Human umbilical cord blood CD34+cells were cultured in Col-Tgel for 3 days as single culture group. MSCs-GFP and human umbilical cord blood CD34+cells were co-cultured in Transwell chamber for 3 days as two-dimensional culture group. Human umbilical cord blood CD34+cells were cultured routinely as control group. The percentage of CD34+CD38-CD45RA-CD90+cells in each group was measured by flow cytometry. In situ immunofluorescence staining was used to detect the activity of cells that were co-cultured in Col-Tgel.(2)In vivo transplantation:NOD/SCID mice subjected to 24-hour X-ray irradiation were divided into two groups: in experimental group, MSC-GFP cells were resuspended in Col-Tgel and transplanted into the tibia of NOD/SCID mice; in control group, MSCs-GFP were resuspended in PBS and transplanted into the tibia of NOD/SCID mice. The MSC-GFP growth in the bone marrow was detected by two-photon/confocal microscopy at 3 days post transplantation. RESULTS AND CONCLUSION: (1) After co-culture in Col-Tgel for 3 days, the percentage of CD34+CD38-CD45RA-CD90+cells in the three-dimensional culture group was 2.8 times that of the two-dimensional culture group, indicating that the MSCs significantly promoted the expansion of CD34+CD38-CD45RA-CD90+cells in the Col-Tgel. The percentage of CD34+CD38-CD45RA-CD90+cells in the three-dimensional culture group was increased by 4.5 times compared with the single culture group and increased by 1.5 times compared with the control group. Immunofluorescence staining showed that the cell viability of human MSCs and human umbilical cord blood CD34+cells was not affected after co-cultured in Col-Tgel for 3 days.In the in vivo transplantation experiment,MSC-GFP cells could survive in the medullary cavity.In summary, Col-Tgel provides a new strategy for stem cell culture and in vivo growth by forming a three-dimensional system similar to the physiological environment in vivo.

ABSTRACT

Objective To discuss the effect of Yanhuning Injection combined with amoxicillin and clavulanate potassium in treatment of infantile pneumonia.Methods Totally 1 00 infants with pneumonia were selected.They were randomly divided into two groups,the observation group (52 cases) were given Yanhuning Injection combined with amoxicillin and clavulanate potassium.The control group (48 cases) were given Yanhuning Injection.The effect of Yanhuning Injection combined with amoxicillin and clavulanate potassium in treatment of infantile pneumonia was evaluated using efficacy,cough,pulmonary rales disappeared time,pyretolysis time,pulmonary function indexes and adverse reaction during treatment as indexes.Results The effective rate was 92.3% in the observation group and 75.0% in the control group.The effective rate in the observation group was higher than that of the control group (P < 0.05).The cough,pulmonary rales disappeared time of the observation group was shorter than that of the control group (P < 0.05).There was no statistical significance on pyretolysis time between two groups.Before treatment,there was no statistical significance on FVC,FEV1 and FEV1/FVC% between two groups.After treatment,the FVC,FEV1 and FEV1/FVC% were increased and higher in the observation group (P < 0.05).During treatment,there was no statistical significance on adverse reaction between two group.Conclusion Yanhuning Injection combined with amoxicillin and clavulanate potassium has a good effective on infantile pneumonia.It could improve the symptoms of pneumonia and lung function while inhibiting inflammation with good safety.It was worthy of clinical application.

ABSTRACT

Objective To explore the application of continuing nursing model in life of puerperae with preterm infants and evaluate its effects.Methods Based on continuous nursing model of Ahmadi,puerperae's continuing nursing program was constructed.Randomized controlled trail design was used,and totally 110 puerperae in a hospital in Beijing were recruited from August 2016 to March 2017.The experimental group received continuing nursing intervention model,and the control group received routine nursing care.Parenting knowledge and psychological evaluation of the two groups were collected 3 days before discharge,1 month,3 months and 6 months after discharge.Results Ninety-eight puerperae completed the study.In the experimental group,the score of parenting knowledge was higher than that of the control group(P<0.01),and the total score of mental health assessment and scores of depression and anxiety were lower than those in the control group (P<0.05).Conclusion Puerperae's continuing nursing program based on the continuous nursing model of Ahmadi improved maternal ability and positive emotion,and promoted quality of life.

Objective The objective of this study is to investigate the effects of the thickness of the adhesives (3M ESPE RelyX ARC) on the internal stress distribution of the IPS Empress II full-ceramic crowns using Finite Element Analysis (FEA). Methods A dummy Empress II ceramic crown restoration of the mandible right first molar was prepared according to standard dental process. Followed by micro-CT scanning, four 3D numerical models with cement thickness 60, 90, 120 and 150μm were established. The models were subjected to four loading conditions and stresses in veneer and core layers were presented. Results Numerical results indicate that when adhesive thickness increases from 60μm to 90μm, the maximum principal stress either in veneer or core decreases. However, when thickness increases to 150μm, stress variation trends differ from adhesives. Conclusion The normal stresses in adhesives remain a low level when the thickness varies from 90μm to 120μm, while the shear stress is less sensitive to the thickness when it exceeds 90μm. There is an optimal thickness which can reduce the tensile stress in the core and veneer. Attention should be paid to the shear strength of the adhesives since the shear stress could cause failure in the adhesive layer.

ABSTRACT

Objective To investigate the application of continuous blood purification in the children with critical diseases. Methods Eighteen critical patients aged 1 ~ 15 years in PICU underwent continuous blood purification(CBP). Fourteen with acute renal failure (ARF) were treated with continuous veno-venous hemmofiltration(CVVH) ,2 with Guillain-Barre syndrome and 2 with Raye's syndrome were treated with plasma exchange(PE). The changes of clinical symptoms, blood biochemistry , blood gas, and oxygenation were analyzed before and after CBP. Results After CVVH treatment, the BUN and creatinine of 14 patients with ARF were decreased from (48.6 ± 14. 8) mmol/L to(28. 9 ±5.4) mmol/L and (634. 3 ±258. 2) μ mol/L to (318.4 ± 143.5) μmol/L,K+ and pH of serum were maintained in the normal range,oxygenation was significantly improved. Breathing difficulties and muscle strength in 2 patients with GBS were ameliorated and successfully weaned from ventilator after PE. Serum ALT,AST and ammonia of 2 cases with Raye's syndrome decreased significantly and they discharged after comprehensive treatment including PE. Bleeding in puncture region were found in 3 patients, hypothermia in 2 patients. During the treatment, vital signs of patients were stable,blood pressure and pulse were not fluctuated. Conclusion CBP is an effective and safe method in the treatment of critical diseases in children.

ABSTRACT

Objective To investigate the application of blood purification for critical disease with non-renal indications in PICU. Methods We retrospectively analyzed the clinical data of 10 critical disease cases with non-renal indications in PICU admitted from Jan to Dec 2009. Five cases were with acute liver failure,2 with autoimmune disease (1 with Guillaln-Barre syndrome, 1 with systemic juvenile rheumatoid arthritis with macrophage activation syndrome) ,2 with severe sepsis,one with metabolic diseases. Results Four cases were treated with plasma exchange combined with continuous veno-venous hemmofiltration. Three cases were treated with continuous veno-venous hemmofiltration. Three cases were treated with plasma exchange.Conclusion CBP is an effective and safe method in the treatment of critical diseases with non-renal indications in PICU.

ABSTRACT

<p><b>OBJECTIVE</b>Leptin (LEP) is mainly produced by white adipose tissue and participates in the energy metabolism and regulation of growth. Cooperating with the other metabolic hormones, it plays an important role in the developments of fetus and neonates. This study was designed to test the serum levels of LEP, neuropeptide Y (NPY), insulin (INS) and insulin-like growth factor-1 (IGF-1) and measure the body mass index (BMI) and head circumference (HC) at different days of life of premature infants with or without serious diseases and to find the changes of serum levels of LEP as well as NPY, INS and IGF-1, the relationship between those hormones and the changes of body weight and the influences of diseases on the levels of those hormones in premature infants.</p><p><b>METHOD</b>The clinical data as well as weights, lengths, HC of 40 sick premature infants (sick group) and 30 premature infants without any diseases (control group) were collected and the serum levels of LEP, NPY, INS and IGF-1 were determined by using radioimmunoassay (RIA) at d 1, d 7 and d 12 of life. BMI was calculated by weight (kg)/length (m)(2). SPSS13.0 was used to analyze the data</p><p><b>RESULT</b>(1) In sick group the serum LEP levels were 0.74 +/- 0.21, 0.60 +/- 0.18, 0.82 +/- 0.12 (mg/L) (P < 0.01), the BMI were 9.81 +/- 1.24, 8.36 +/- 0.87, 9.08 +/- 1.12 (kg/m(2)) (P < 0.01) on d 1, d 7 and d 12, respectively. In control group serum LEP levels were 0.78 +/- 0.17, 0.71 +/- 0.17, 0.88 +/- 0.58 (mg/L) (P < 0.01), the BMI were 10.03 +/- 1.04, 9.35 +/- 0.80, 11.06 +/- 0.82 (kg/m(2)), on d 1, d 7 and d 12, respectively (P < 0.01). In both groups, serum LEP levels as well as the BMI decreased on d 7 and reincreased on d 12. The differences of serum LEP levels and BMI between sick group and control group at d1 were not significant (P > 0.05); compared with control group, the serum LEP levels and BMI on d 7 and d 12 in sick group were lower and the differences were significant. (2) There were positive correlations between serum LEP levels and BMI in sick group as well as in control group. (3) In sick group, the serum NPY levels at d 1, d 7, d 12 were 55.33 +/- 9.38, 46.64 +/- 6.17, 75.13 +/- 9.12 (ng/L) (P < 0.01), INS were 10.07 +/- 2.63, 7.71 +/- 2.77, 10.37 +/- 2.29 (mU/L) (P < 0.01), IGF-1 were 38.66 +/- 11.42, 31.98 +/- 7.34, 41.84 +/- 8.05 (mg/L) (P < 0.01), respectively. In control group, the serum NPY levels at d1, d 7 and d 12 were 57.77 +/- 7.15, 48.49 +/- 8.81, 81.36 +/- 8.51 (ng/L) (P < 0.01), INS were 11.55 +/- 1.99, 8.28 +/- 2.87, 15.42 +/- 3.80 (mU/L) (P < 0.01), IGF-1 were 37.76 +/- 7.07, 34.33 +/- 8.97, 50.19 +/- 8.38 (mg/L) (P < 0.01), respectively. In both groups, serum levels of NPY, INS and IGF-1 had positive correlations with serum LEP levels as well as BMI on the corresponding days and decreased on d 7 and reincreased on d 12.</p><p><b>CONCLUSION</b>(1) The serum LEP levels decreased on 7 d of life and reincreased on 12 d of life, which corresponded to the changes of the physical development of premature infants. (2) The serum LEP levels in sick premature infants decreased definitely as compared with control group, which suggested that diseases had negative influences on the LEP levels and the physical developments were slowed down in sick premature infants. (3) The serum levels of NPY, INS and IGF-1 had positive correlations with LEP levels as well as BMI at the early period of life, which suggested that NPY, INS and IGF-1, cooperating with LEP, might take part in the regulation of development of premature infants.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To establish the rabbit VX2 tumor as a model for pyriform sinus carcinoma and to observe its biological features.</p><p><b>METHODS</b>VX2 tumor was implanted into the pyriform sinus of 15 rabbits by direct laryngoscope. Fifteen rabbits were randomized into 3 groups (average of 5 rabbits per group). Observation of the tumor growth and evaluation of the histopathological characterization were taken on one group each at the of time 14, 21 and 28 days after transplantation respectively.</p><p><b>RESULTS</b>Tumors were found grown in the pyriform sinus of all 15 rabbits with a success implantation rate of 100%. Dysphagia, body weight loss, rhinorrhea and short of breath appeared in the rabbits 28 days after transplantation. The metastasis rates of deep cervical were 100% in all three groups. The metastasis rates of submandibular lymph nodes were 3/5, 4/5 and 5/5 in 14-day, 21-day and 28-day group respectively. The metastasis rates of paratracheal lymph nodes were 0, 4/5 and 5/5 in 14-day, 21-day and 28-day group respectively. There were opposite side lymph node metastasis of deep cervical, submandibular and paratracheal in 4, 3 and 5 rabbits on 14, 21 and 28 days after transplantation respectively. The median diameter for deep cervical, submandibular and paratracheal neck lymph nodes were 1.50, 0.60 and 0.0 cm in 14 days; 1.60, 0.80 and 0.50 cm in 21 days; 1.80, 0.8 and 0.65 cm in 28 days (P > 0.05).</p><p><b>CONCLUSIONS</b>The animal model for pyriform sinus carcinoma is established successfully. The metastasis of deep cervical lymph node could be induced from day 14 after VX2 transplantation.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To analyze the stress distribution in all-ceramic crowns when it was subjected to load.</p><p><b>METHODS</b>A 3D numerical model of the all-ceramic crown of the right mandibular first molar was generated from scanned CT images. Finite element analysis (FEA) was used to evaluate the stress distribution in the all-ceramic crown when it was subjected to 5 load conditions.</p><p><b>RESULTS</b>Stress distributions under 5 loading conditions were obtained. Stress concentrations were generally found near the loading areas at the veneer and at the lower surface of the core beneath the loading areas. In the 5 loading conditions, it was found that when the crown was loaded with vertical concentrated load, the tensile stresses around the shoulder areas were uniform, while stress concentration with maximum of 32.25 MPa was found at the shoulder areas in lingual-buccal direction when loads were applied at an angle of 10 degrees with the tooth axis on the buccal side. The masticatory load which was applied at 20 degrees with the tooth axis on the buccal side would cause stress concentration at the shoulder in mesial-distal direction. The maximum value could reach 11.29 MPa.</p><p><b>CONCLUSIONS</b>The occlusal surface of the all-ceramic crown must be trimmed to increase multiple contact zones with the opposite surfaces in antagonist teeth to avoid excessive concentrated stress that may cause crown failure.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To report clinical application of Extracorporeal membrane oxygenation for severe acute respiratory and heart failure in a child with severe pneumonia.</p><p><b>METHOD</b>A seven-year old male patient with severe pneumonia complicated with heart and lung function failure was admitted to PICU in 28th of December, 2008.Veno-artery access was set up via euthyphoria cannulation in operative incision. Blood was drained from the right atrium through a cannula introduced via femoral veins, and returned via femoral artery. The inter-surface of the ECMO equipment system was completely coated with heparin-coating technique. Anticoagulation was maintained with heparin to keep the activated clotting time (ACT) between 150 and 200 seconds and heparin usage dose was 10 U/(kg.h), mean blood flow was 1/2-2/3 of 80-120 ml/(kg.min) during ECMO assistant period. During ECMO, ventilator settings were gradually reduced to allow lung rest, i.e. peak inspiratory pressure less than 25 cm H2O (1 cm H2O=0.098 kPa), end expiratory pressure 8-10 cm H2O, rate 10-15 breaths per minute and FiO2 30%-40%.</p><p><b>RESULTS</b>In management of ECMO, the incipient blood flow was set at 0.8 L/min, the radio of oxygen and blood flow was 1:1, FiO2 60%. After ten minutes of ECMO working, the blood oxygen saturation of radial artery increased from 40 mm Hg (1 mm Hg=0.133 kPa) to 177 mm Hg, Lac decreased from 3.5 mmol/L to 2.8 mmol/L. Four hours later, blood gas analysis of radial artery showed PaO2 202 mm Hg, PCO2 44 mm Hg, Lac 1.5 mmol/L, blood flow was set at 0.6 L/min, FiO2 60%, PaO2 kept above 150 mm Hg. 96 hours after ECMO supporting, the blood flow was set at 0.4 L/min [20 ml/(kg.min)], the results of blood gas analysis of radial artery was PaO2 190 mm Hg, PaCO2 36 mm Hg, SaO2 100%, Lac 0.9 mmol/L, then the child weaned off successfully from ECMO. Two days later, the child was successfully extubated. After two weeks treatment, the patient was discharged. The main complication associated with extracorporeal membrane oxygenation were bleeding.</p><p><b>CONCLUSION</b>ECMO is an effective mechanical assistant therapy method for severe pulmonary and cardiac failure in a child.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effects of orthodontic treatment on temporomandibular joint (TMJ) in adult low-angle patients with skeletal class III malocclusion.</p><p><b>METHODS</b>Sixteen adult female low-angle patients with skeletal class III malocclusion were included in the study. All patients were treated with OPA-K straight wire technique. Lateral cephalometric and corrected transcranial projection films were taken before and after treatment. Cephalometric analysis was carried out. All data were analyzed statistically.</p><p><b>RESULTS</b>After orthodontic treatment, the linear measurement and area of anterior space in TMJ increased by 0.27 mm (P < 0.001) and 0.70 mm(2) (P < 0.01), respectively, while the posterior space decreased by 0.24 mm and 0.67 mm(2), respectively (P < 0.001). Linear ratio decreased from 22.13% to 9.64% (P < 0.001), and area proportion decreased from 1.56 to 1.19 (P < 0.01). Anterior space became equal to the posterior space. Cephalometric analysis showed that point Co, Ar, Go and Pg were all retruded (P < 0.05).</p><p><b>CONCLUSIONS</b>After orthodontic therapy, the condyle moved posteriorly to normal concentric position, which would be helpful in alleviating temporomandibular disorders (TMD) symptom in adult low-angle patients with skeletal class III malocclusion.</p>

ABSTRACT

<p><b>OBJECTIVE</b>The aims of the present study were to assess the characteristics of MSCs which were induced and proliferated in vitro, then to learn the interfaces between them and metal materials.</p><p><b>METHODS</b>Adult dogs were selected as experimental animals, the MSCs (marrow stem cells) were aspirated from the femur, which were induced to the phenotype of OB (osteoblast), then to proliferated in vitro with metal materials. Using biochemical tests, phase contrast microscopy, transmit electromicroscope, scanning electromicroscope to detect MSCs and the image of interface.</p><p><b>RESULTS</b>MSCs had powerful osteogenic phenotype when they were induced in vitro, and they also attached to the metal materials. And there were more cells which attached on tensile stress area than other areas.</p><p><b>CONCLUSION</b>The MSCs induced in vitro could be used as source of osteoblast. The affects of air--abrasion with alumina are not significant, but they have relation to the extent of strain and property of stress.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the clustering character of overweight and obesity with multiple cardiovascular disease risk factors such as blood pressure, serum lipids and glucose, and to provide evidence for intervention in childhood.</p><p><b>METHODS</b>A cross-sectional study was conducted on 913 children aged 7-13 years in urban area of Shijiazhuang. Measurements included height, weight, systolic blood pressure (SBP), diastolic blood pressure (DBP) and fasting serum total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C) and serum glucose (GLU).</p><p><b>RESULTS</b>Overall prevalence rate of overweight and obesity was 29.24% (boys: 34.98% vs. girls: 24.23%). When compared with normal children, children with overweight or obesity had significantly higher levels of SBP, DBP, TC, TG but low HDL-C. The prevalence rates of high SBP, DBP, TC, TG, GLU and low HDL-C in overweight and obese children were higher than in normal children. After adjusted for gender and age, the odds ratios of overweight and obese children for high SBP, DBP, TC, TG, GLU and low HDL-C were 6.77, 3.22, 2.55, 6.42, 3.85 and 2.94 (95% CI: 4.15-11.04, 1.38-7.49, 1.59-4.11, 3.46-11.92, 1.69-8.78 and 1.83-4.73), respectively. Odds ratios of overweight and obese children holding any one, two or three of the selected six risk factors appeared to be 2.74, 13.15 and 15.33 (95% CI : 1.92-3.92, 6.69-25.87 and 4.17-56.39), respectively.</p><p><b>CONCLUSION</b>Childhood overweight and obesity increased the clustering of children's risk factors on cardiovascular diseases, as well as increasing the risk of children acquiring multiple cardiovascular disease risk factors. Controlling overweight and obesity would help early prevention on children from getting cardiovascular diseases.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the usage of moiré interferometre in porcelain-fused-metal interfacial fracture mechanics.</p><p><b>METHODS</b>Raster was produced on the surface of samples with different thickness of porcelain and metal in 5 groups. The stripes of u field and v field under different load were collected. Then the rupture tenacity Jc was calculated and the extensions of flaws were observed.</p><p><b>RESULTS</b>Stress concentration appeared around the flaws under load conditions. The stripe became denser under more load. The rupture tenacity Jc and the direction of flaw extension of each group were different.</p><p><b>CONCLUSION</b>Moiré interferometre can be used in the study of porcelain-fused-metal interfacial fracture mechanics and to prognosticate the direction of flaw extension.</p>

ABSTRACT

Objective To investigate the influence of intermittent catheterization methods on the urinarytract infection in patients with spinal cord injury. Methods Thirty-one cases of spinal cord injury and urinarytract infection were recruited. By use of intermittent catheterization, the bladder function was trained. Bacteria cul-ture and identification of in urine from the patients were conducted before the training (indwelling catheterization)and after 15 or 30 days of intermittent catheterization.
